Patient Paperwork
Since our move to North Logan, we have revised and updated our patient paperwork. We will provide you with our updated policies at your next appointment or upon request.
Paperless Statements and Letters
Introducing paperless statements and letters! Snail mail is going out of style. If we already have an email on file, you don't need to change anything. Expect to start seeing regular statements during the first week of each month, beginning February.
Medication Refills
We are enforcing our new policy that medication refill requests must be done at least 3 business days in advance. We cannot always guarantee an immediate response to every request. Please be proactive and request a refill with anticipation before you are close to running out of medication. Controlled substances will not be refilled without an appointment.
